Here is a list of the information/documents you should bring with you on your first visit

• Immunization records
• Growth charts
• Medical Records Summary
• Name and phone # of previous physician
• Child(ren)'s Insurance Card(s), or other proof of coverage
• List of current medications and doses
• List of allergies
